A fixed point of pain in the lower right abdomen that worsens with the urge to pass stool and has been present for 6 years without any fever or vomiting is most consistent with IBS-related pain, likely involving the ileocecal region. Appendicitis would not last 6 years without becoming an emergency, so that is very unlikely. However, a small inguinal hernia should also be ruled out.
Next Steps

Hi!
Pain in the lower right abdomen triggered by bowel movements is due to IBS or intestinal spasms, especially with your history. 😊
The long duration without fever or vomiting is makes acute issues like appendix less likely.
Feel free to consult for a detailed check and tailored relief. 👍
