you need to get a root canal treatment done.make sure the dentist and you both are free from any covid symptoms. once your tooth is opened you will get relief. antibiotics and painkillers will work for a short time only and if delayed could lead to a cyst formation.

Carious process in first premolar is reaching the pulp .if it is tender on clinical examination.then root canal is needed.For PDL widening prophylaxis is needed which can be done only after Covid.

Hello Sir , Welcome to Practo . I have gone through your query and the xray attached .  Nothing to worry ,you will ne needing a Root Canal treatmnt followed by a cap . You will have to visit the Dentist  3-4 times (sittings) and every things will get resolved.  You might be in pain  as the caries is  near pulp . Tab Mox 500mg 1 tab td x 5 day Tab Zerodol Sp 1 tab Td x 5 days Cap Pan D 1 Cap bd before meal x 5 days . Warm saline gargle 2-3 times a day . Avoid if possible for a Dental visit due to Covid 19 . The x ray shows dislodged restoration reaching  till pulp cavity...u are advised to opt for indirect pulp capping followed by restoration /rct depending  upon pain after the procedure

From xray we can see deep filling in premolar which is very close to the nerve fibers and putting pressure on it..if you are not allergic to any medicine then take Tab Zerodol p twice daily for three days Tab augmentin 625mg twice daily for three days Cap biglac twice daily for three days

Problem is with your previously filled tooth(premolar). You need to remove the filling to relieve pressure of tooth. N then root canal treatment of tooth with cap Now due to continuous irritation you must be feeling pain over their.

Rx 1.Cap Moxikind CV 625 (10) 2times in day for 5 days after meal 2.Tab Zerdol SP (10) 2times in day for 5 days after meal 3.Tab pan 40 (5) 1time in day for 5 days before meal 20min

The root end is cut off upper 1st premolar. So presence of any periapical pathology can't be elicited Depending on the depth and extent of secondary caries related to the restoration. It has to under go RCT. Due to the covid pandemic, you can still delay for a month or 2. But you need to be very careful with the tooth.you should eat any hard food at all, which may lead to fracture of restoration as well as the tooth structure. AVOID HARD/VERY HOT/ COLD /SWEET/STICKY food. Maintain soft diet.of normal temperature. Brush thoroughly twice a day,after meal. Use dental floss to avoid further food lodgement in between teeth 2-3 times/day regularly. For the time being, do have these medicines prior to visit dentist, T. augmentin duo 625, 3 times/day.after food 10 tablets. T.metrogyl 400, 3 times/day.after food 10 tablets. T.ultracet/zerodol PT, maximum 3 tablets a day, if pain,after food T. Roles D,once a day, before food,for 4 days T.A to z gold, once a day, after food,10 tablets. If feverish, that can be as a secondary  infection from tooth.

visit dentist and get RCT done after 2 months. 1-2 month's delay won't harm if the other precautions are well kept in mind and maintained accordingly.

do not do hot compression /balm/ointment application extra-orally. avoid very hot/cold/hard/sweet/sticky food soft diet do not neglect brushing after meal. hold mouthful of Luke warm salt water in for sometime; you can do it for few times a day, if it's relieving your pain, even partially.

The filling done in this tooth has reached pulp and that is causing pain to you..
