Persistent upper abdominal pain with a heterogeneous liver appearance and abnormal liver enzymes needs correlation with the full report and examination.
Fatty liver is common, but gallbladder, gastric, muscular and other causes may also produce pain on either side.
Avoid alcohol and unnecessary painkillers/supplements, eat lighter meals, and continue only medicines advised after review.
Seek urgent care for severe worsening pain, fever, repeated vomiting, jaundice, black stools or abdominal swelling.
Please book a consultation with the ultrasound images and complete liver tests so further testing and treatment can be planned.

Your GGT (182) and ALT (119) are elevated and, along with a heterogeneous liver on ultrasound, warrant further evaluation.
This does not by itself mean serious liver damage, especially with normal AST, but recurrent pain for 3–4 months should not be managed only by repeatedly taking medicines.
Please avoid alcohol and unnecessary OTC/herbal supplements and get a repeat LFT with bilirubin, ALP, albumin and PT/INR, along with hepatitis B/C screening.
Depending on the examination and reports, further tests or additional liver work-up may be needed.
Please consult me on Practo with your complete LFT and ultrasound reports so I can assess the cause of the pain and abnormal liver enzymes and guide you further.
Needs further evaluation rather than continuing the same medicines.
A consultation with repeat LFTs and review of the ultrasound is advisable to identify the cause and guide treatment.
it would have been nice if you have included whether its continuous pain or intermittent pain.....in the picture you have marked, it includes both lower chest and upper abdomen.
you have sorted upper abdominal causes by getting USG abdomen and consultation with gastroenterologist. the medicnes that are prescribed are right and in right dose and frequency. along with heterogenous echo texture in liver they would have mentioned which grade fatty liver it is. that could have helped us to understand your current condition better.
we still need to ruleout lower chest causes for pain
