According to the report. You need extraction of all the 4 wisdom teeth. And other two teeth we need to see the X-ray. If there is enough tooth structure can go fr root canal followed by cap.

Hi, After reading your x-ray report, advise extraction of all 4 wisdom teeth. There are 2 teeth that have decay, need to see the x-ray to know how much tooth structure is lost to advise the treatment option.
