Hi, Pain in hand and foot joints are generally due to RA, but she might also have a nerve compression in her back due to which she is experiencing pain in her shoulders and hands. Will require some blood and radiological investigations to know the exact cause so treatment can be started accordingly. Consult directly so that we can get a further workup and start the treatment accordingly

Most likely the pain is due to active rheumatoid arthritis. Alternatively, there may be nerve compression along with it. Diagnosis depends on a detailed history and relevant investigations.
Next Steps
Consult a Rheumatologist (arthritis specialist) for correct diagnosis and treatment
