In coronary blockades the myocardium distal to blockade suffers ischaemia and so the cells are deprived of their normal quota of oxygen in absolute terms despite general SpO2 being normal which is a percent saturation level of hemoglobin. Heart may suffer hypoxia due to blocked coronary (ischaemia) despite blood having normal saturation with oxygen.
