Screw fixation at 70 yrs is not at all indicated. Your choices are between bipolar or total hip. If the patient is moderate build, community ambulator and active then total hip would be the choice. But if patient is frail and only home ambulator the bipolar would be better. Moreover copd is another factor in support of bipolar. Nowadays bipolar implants are coming good which last long.
Next Steps
visit a hospital with icu setup for surgery. do not delay. earlier the surgery better is the prognosis
