According to my hunch is that you are feeling depress and anxiety. Depression can be about failure, loss or undeserved plight and depression is past Focus but anxiety is future focus about some threat .In depression and anxiety we rumminate lot . In depression we find ourself in hopeless and helpless condition , withdraw from enjoyable activities and in anxiety we have thoughts of incapability, unable to deal with threat ( problem) , overestimate problem and underestimate our capabilities. so you need to find about what  are you feeling depress and anxious, after this you  have to change your attitudes so that you don't feel depression or anxiety.
Next Steps
go for counselling

You are 23 and your difficulties started when you were 19 or so. When problems of these kinds trouble you for such a long time and do not go away despite your best efforts and intentions, it should probably give you an indication that you need to take professional help, in this case of a psychiatrist. Prolonged anxiety or depression do not go away just by brushing the symptoms under the carpet. If you need to see a proper doctor for your condition, you have to, because if you don't, you will continue to suffer. Once the psychiatrist deals with your persistently depressive condition of mind, probably through medication, you need to consult a psychologist for working out strategies to ensure that your condition does not return. This is how you need to go about, I think. Our social and family environments often discourage people from seeking help from a psychiatrist. If you too have such an environment around, and your mindset too prevents you from seeking professional help, please visit a psychologist first and get guided on how to go about visiting a mental health doctor.

I'm sorry to hear that you're going through this. It sounds like you're dealing with persistent thoughts and fears that are affecting your well-being. It's essential to acknowledge the impact this has on your daily life. Seeking support from a mental health professional, like a counselor or therapist, can provide valuable strategies to manage obsessive thoughts and fears. They can work with you to develop coping mechanisms and offer a safe space to explore and understand these concerns.
