If your Son is active, eating well and sleeping well it's completely fine. And no any symptoms like tiredness, loss of appetite and no stamina than it's a problem. Firstly take up home fitnessbtests which he can do for himself -balancing tests(one leg balance for a min) - Flexiblity( sit and reach) -Muscular strength(push-up) - Cardio endurance ( 2km run) If he passes all this then he is super healthy , if required improvement w need to start working on strength training or running , that will indirectly increases his metabolism and food intake as well. This method improve his fitness level
Next Steps
Eating high calorie nutritious diet will also help of he is Under weight. If weight is correct for his age then nothing to worry.
