Crocin - 8 mL can be given and meftal P 6.5 mL. Meftal syrup to be given only if there is high grade fever more than 102F in SOS basis. You can continue with Crocin itself every 4-6 hrly. Tepid sponging would also be helpful. If you want to give both the drugs maintain Atleast 4-5 hrs gap if the meftal syrup you are using has paracetamol.

For a child weighing 27 kg, the doses mentioned appear reasonable provided the formulations are Meftal-P 50 mg/5 mL and Crocin 240 mg/5 mL. However, medicines should ideally be dosed according to the child’s age, weight and clinical condition.
Meftal-P (mefenamic acid) and Crocin (paracetamol) do not need to be routinely alternated. If your pediatrician has specifically advised alternating them, Crocin can generally be given when the next dose is due as instructed; there is no requirement to wait for the fever to come down completely. Do not give repeated doses more frequently than prescribed.
A temperature of 101°F after 3 hours does not necessarily mean that the medicine has failed. The aim of fever medicine is primarily to improve the child's comfort, rather than to bring the temperature immediately to normal.
Please ensure adequate fluids and monitor the child. If the fever is persistently ≥103°F, lasts more than 2–3 days, or the child is unusually sleepy, difficult to wake, breathing rapidly/difficultly, has persistent vomiting, seizures, severe headache/stiff neck, rash, poor urine output or appears very unwell, the child should be examined urgently.
Since this is a child, I would also recommend confirming the exact age and the strength written on both bottles with the treating pediatrician before continuing the alternating schedule.
Hello. Between 2 doses of Mefthal P minimum of 8 hours gap is required. For Crocin 4-6 hours gap is ok. If Mefthal you are referring does not have paracetemol you can have 2-3 hours gap between two medicines. In between you can give sponging for the child with lukewarm water. Doses seem ok. Kindly take the child for physical consult to check him for other signs.
Greetings
Primary Medication Schedule
Crocin (Paracetamol): Give 8 ml every 6 hours as the primary treatment for the fever.
Emergency / High Fever Medication (SOS)
Meftal: Give 6.5 ml only as needed (SOS basis) if the child's temperature reaches 102°F or higher. This medication should be administered under the guidance of a pediatrician.
Home Care and Supportive Tips
Hydration: Provide plenty of oral fluids to keep the child well-hydrated.
Cooling: Use tepid sponging (sponging the body with room-temperature water) to help safely lower the temperature.
Next Steps
Seek an online consultation or follow up with your pediatrician if the fever persists or if you need further guidance.
Doses to be given:
Crocin - 8ml
Meftal - 6.5ml
Primary drug for fever remains paracetamol. Meftal should be given on SOS basis with the guidance of a pediatrician.
Give paracetamol 6th hrly .
Meftal can be given if the temperature is 102 or above
