I have noticed this several times: whenever I take creatine (2 g), my lower back becomes very stiff. The stiffness extends from my buttocks to my lower back, and I have difficulty bending forward. This happens even when I have not done any weight lifting or strenuous exercise. Could creatine be triggering this stiffness?

Answer to your question is YES. It can trigger. Some users of creatine report stiffness or cramps, often linked to low fluid intake. Make sure you have adequate hydration. That itself will resolve your issue.

Creatine itself does not cause lower back stiffness. What it does is pull water into muscle, so if your fluid intake is low you get tight, cramp-like muscles - and with HLA-B27, ESR, CRP and RF all normal, inflammatory back disease is effectively ruled out. Your pattern sounds mechanical: tight hamstrings/glutes and weak core, worsened by dehydration. Drink 3-3.5 litres water daily while on creatine, split the dose, and do daily hamstring plus hip flexor stretching and core work.
