Lower abdominal pain radiating to legs
I had chronic fissure & bleeding for 6 years . So I underwent kshara sutra thinking it's less painful. But the thread changing sessions went on for 2 months & even after that there was lot of pain & didn't seem like it was healing. So I underwent another fissure tissue removal surgery 4 months ago. I had followed everything doc had told. I was getting better. But I have had pain & can't manage a day without 2 sitz bath. Moreover even after following all that, I've been having a strange lower abdominal pain & further the whole lower body starts paining. More like pulling & burning .I even have the anal pain. There's no bleeding or pus. But pain.I really have no energy to undergo another surgery. This is my 5th surgery & I have other health probs too. Already lost 20kgs coz of previous surgeries.Feeling more weak now. Also want to mention that in college, i had eaten lot of slate pencils for 3 years. Is that the reason? Now i don't eat. I can't quit my job too. Please help & guide 😭 painful

Consult the colorectal surgeon/gastroenterologist soon and explain the 20-kg weight loss and severe weakness. * Ask the doctor to evaluate for anemia, nutritional deficiencies, infection/inflammation and other causes of weight loss. * Useful tests may include CBC/Hb, ferritin + iron profile, B12, folate, vitamin D, electrolytes, albumin/total protein, CRP/ESR, LFT/KFT, and additional tests based on examination. * Because the fissure is recurrent despite multiple surgeries, the doctor should also reassess the diagnosis and look for underlying causes such as Crohn’s disease or other bowel disorders, particularly if there is diarrhea, abdominal pain, fever or unusual/multiple fissures. For weakness, focus on small, frequent, calorie- and protein-rich meals rather than large meals. About eating slate pencil Yes, it is important to mention this to the doctor. Eating slate pencil is a form of pica, which can be associated with iron deficiency/anemia. It may therefore be a sign of an underlying deficiency rather than the reason for the 20-kg weight loss.

Please consult a colorectal surgeon/gastroenterologist for the ongoing anal, lower abdominal and leg pain before considering another surgery. Do not ignore persistent or worsening pain, especially with fever, swelling, vomiting, urinary problems, or new bleeding. Focus on soft, easy-to-digest, nutritious meals and avoid skipping meals. Include adequate protein: eggs, curd, milk, dal, paneer, fish or chicken as tolerated to support healing and regain strength. Include fiber gradually from vegetables, fruits, oats and whole grains, along with enough water, to keep stools soft. Avoid constipation and straining during bowel movements. Continue sitz baths only as advised by her doctor. Since you  lost around 20 kg and feels weak, your calorie and protein intake should be assessed and increased gradually. Eating slate pencils for years can sometimes be associated with iron or other nutritional deficiencies, so you should discuss checking CBC, ferritin/iron profile and other deficiencies with your doctor. Do not start iron or other supplements without checking her blood reports/doctor's advice.
Health Tips
Diet can support healing, but your persistent pain needs proper medical evaluation rather than trying another procedure or managing it only with diet.

Please consult your colorectal surgeon/proctologist again, preferably for a second opinion from a colorectal specialist, and discuss the persistent pain and the new abdominal/lower-body symptoms. Your doctor may need to assess whether there is ongoing fissure-related inflammation, muscle spasm, infection, nerve-related pain, or another underlying cause. If the pain is severe or worsening, or you develop fever, vomiting, abdominal swelling, difficulty passing stool/urine, pus, or significant bleeding, please seek urgent medical care. ☆ For now: • Avoid crash diets, fasting, or unnecessarily restrictive diets. • Focus on small, frequent, nourishing meals to gradually restore your energy and weight. • Include adequate protein from well-tolerated foods such as curd, paneer/tofu, eggs, fish/chicken, or other suitable sources according to your tolerance. • Maintain good hydration. • Aim for soft, fibre-containing foods and avoid constipation/straining, but increase fibre gradually and according to your doctor’s advice. Regarding eating slate pencils during college, can sometimes be associated with iron or other nutritional deficiencies, although it cannot by itself explain your current symptoms.
Next Steps
You do not necessarily need to assume that another surgery is the only option. First, please get a proper reassessment and second opinion so that the cause of the persistent pain can be identified and the least invasive appropriate treatment can be discussed. You’ve been dealing with this for years, so please don’t ignore the new symptoms or continue suffering without reassessment.
