Hello Doctor, I wanted to ask about a private skin issue. I have itching in my groin and pubic area, where there is also hair. Sometimes the itching becomes worse after I use soap there. Could you please advise what I should use and whether I should avoid soap in that area? Also, is it safe to shave the pubic hair, or would that make the itching worse? I've also noticed a small pimple/lump near the buttock crease/opening area, which sometimes causes discomfort when I sit.

I may be fungal infection
So pls clean the area
Clean it with leukwarn water
Use cotrimazole cream
And if it's is excess then take tablet fluconazole 1 weekly for 2 wks
Must be fungal infectiondue to moisture in the groin.remove hair in the area and keep area clean and dry and wear clean cotton inner wear.Apply clotrimazole anti fungal cream 3 times a day.

Nothing embarrassing here - this is everyday stuff in my OPD. Itching in the groin and pubic area that worsens with soap is most often tinea cruris (fungal ring infection) or a soap/sweat irritant dermatitis. Harsh soaps and antiseptic washes strip the skin and make it burn more, so use plain lukewarm water or a mild syndet cleanser, dry the folds properly, and wear loose cotton innerwear. Shaving is safe but do not dry-shave - trimming is safer, since razor nicks cause folliculitis. The small painful lump at the buttock crease is likely an infected hair follicle or early pilonidal boil.

Stop the soap in that area, keep it dry, and get examined so the right antifungal cream and duration is chosen - most people stop too early and it relapses. Do not use steroid-containing combination creams sold over the counter; they make fungus far worse.
