Is percussion sensitive says fx &extract
I have lower jaw pain under the ear lobe for more than 2 years. Dentist spotted the tooth 46 hurts for tapping and biting pressure on it. Cbct and xrays are clear.Endo did root canal in 2020 for the same symptoms. No relief. Then had partial retreat on distal canal last year from another Endo. No relief. Same symptoms of tapping and biting pressure on it. Cbct taken again and normal. Went back to the same Endo and he reopened the whole tooth in May 2022. When he reopened, he saw mesial old filling was wet, stinks and blood was like purplish. he placed calcium hydroxide inside. It's been 45 days now and it's the same symptoms..tooth still hurts for tapping, pressing or pushing with fingers on it, biting on it. Endo wanted me to see the oral surgeon for Extraction instead of finishing the retreat permanent sealing step.It hurts when they tap on it even with popsicle stick and biting on cotton roll on it.I have crown placed on it in 2020, retreat thru the crown. Do I need to ext it?

Your x-rays are not giving a clear picture. It's a very rare scenario of tooth being tender even after re root canal procedure. Either there are remnants of pulp or either tooth root is vertically fractured which can be seen only in cbct. Pulpal remnants can be infected and cause pain but it should be relieved with a proper re root canal treatment. If pain doesn't subside after a re root canal also.....your tooth might be suffering with vertical root fracture.... extraction is the only option in that case. Consult a good Endodontist for accurate diagnosis and get your treatment done

Hi..through the radiograph,rct is not clear.please consult alternative endodontist and get an opinion, if it doesn't work,get the tooth removed. Pain may be due to improper rct.
