Is DMSA scan mandatory ?
11 months old baby boy is diagnosed with E-Coli UTI infection and he is under IV antibiotics course since 2 days, past 2 days no symptoms of fever, he is taking food and fluids normally, urinated 10 times in a day.. Is DMSA scan mandatory now ?

DMSA scan is done if the treating doctor suspects some pathology in the kidney like stone, tumour etc. In your baby's case it appears to be uncomplicated UTI which would resolve with antibiotic therapy. Discuss with doctor. The cardinal rule is not to do tests unless absolutely necessary, both cost wise and health wise.

DMSA scan in this admission is used to diagnose whether it is a simple cystitis or if the infection has affected kidneys(called pyelonephritis).  DMSA scan needs to be repeated in some patients after 6 weeks to find if infection has damaged kidneys or not.

There are two tests which are normally done. DMSA and a MCU. It depends what the parents and physician are comfortable with. Depending on the clinical situation both may b be required, one after the other. DMSA is comparatively more non invasive and should ideally be done. What is important is not only to resolve this episode but to also prevent further similar episodes. DMSA will look at scarring in the kidney.
