Intermittent pain on the right side
I have been experiencing occasional pain on the right side, below the ribs, for about 3 months. The pain stopped completely for some time but started occurring occasionally again. I had an ultrasound, which showed mild hepatomegaly (mildly enlarged liver). I took two bottles of Adlive, after which the pain improved, but it later returned occasionally. The pain is now less than before. Please advise what could be the possible cause and whether I need any further tests.
Recently had a FibroScan. My report shows CAP 310 dB/m, liver stiffness 4.8 kPa, IQR/Median 22%, and 10 valid measurements. What do these results mean? Do I have fatty liver, and if so, what grade is it? Is 4.8 kPa normal, or does it indicate any liver fibrosis? Do I need any further tests or treatment, and when should I repeat the FibroScan?”

Your Fibroscan actually reads well: CAP 310 dB/m means significant fatty liver (grade S3 range), but stiffness 4.8 kPa is F0-F1, i.e. no fibrosis at all. IQR/median 22% makes it a reliable scan. So the mild hepatomegaly and that dull right-sided pain under the ribs are from fat in the liver, not scarring - and at 20 this is fully reversible with weight and diet, not medicines.
Next Steps
Get LFT, fasting sugar/HbA1c, lipid profile and TSH once, plus hepatitis B and C screening; no need to repeat Fibroscan before 2-3 years unless LFT worsens. Stop Adliv-type liver tonics, cut sugar, maida, fried food and any alcohol, and do 45 minutes brisk exercise daily aiming for 7-10% weight loss.
