25 years is a long time to struggle with sleep. Youâre right to look for something that lasts rather than a quick fix.
The most effective long-term treatment for chronic insomnia is CBT-I, cognitive behavioural therapy for insomnia. Medical guidelines recommend it as the first choice, ahead of sleeping tablets. Tablets often stop working well over time. CBT-I retrains the bodyâs sleep system, so its benefits last after treatment ends. It usually takes 6â8 sessions and works well even for insomnia that has gone on for decades.
CBT-I works on what keeps insomnia going: spending long hours in bed awake, an irregular sleep schedule, the anxious âI wonât be able to sleep tonightâ thoughts, and the bed becoming linked with frustration instead of sleep.
With diabetes, itâs also important to rule out medical factors that disturb sleep:
Frequent urination at night, from high sugar levels.
Low sugar at night, which can happen with some diabetes medicines and cause sweating, waking, or nightmares.
Nerve pain or tingling in the legs.
Sleep apnoea. Loud snoring, gasping, or daytime sleepiness are signs. Itâs common in people with diabetes and often missed.
Next Steps
Discuss these sleep-disturbing factors with your diabetologist. Ask whether a sleep study is needed if you snore. Consult a clinical psychologist trained in CBT-I. It can be done online. Keep a sleep diary for 2 weeks before your first session: bedtime, time it took to fall asleep, number of wakings, wake-up time. This makes treatment faster and more accurate.
For 25 years of insomnia, the priority is to identify the underlying cause rather than relying only on sleeping tablets. Diabetes, sleep apnea, restless legs, anxiety/depression, nocturia, pain or other medicines may contribute.
A durable approach usually includes good diabetes control, sleep-hygiene measures and CBT-I (Cognitive Behavioural Therapy for Insomnia); medicines can be added when clinically indicated. Avoid taking sedatives or homeopathic remedies on your own without proper assessment.
