This is functional constipation, needs patience and regular treatment. Rx: Syp Lactulose (Duphalac / Cremaffin plain) 5 ml at bedtime Toilet routine after meals Fibre + fluids Avoid forceful toilet training Consult a paediatrician for further management if needed.

This is most likely functional constipation, which is very common in 2-year-old children and usually starts after a painful hard stool makes the child start withholding stools out of fear. When stool stays inside longer, it becomes harder and larger, making passage more painful and worsening the cycle. This is not due to a serious disease in most children.
Next Steps
Your child needs regular stool softening for several weeks, not just for a few days. Continue a doctor-prescribed osmotic laxative like lactulose or polyethylene glycol daily until stools become soft and come every day or alternate day. Increase water, fruits, vegetables, and fiber in the diet and avoid excess milk, biscuits, and refined foods. Make her sit on the potty after meals daily to build a routine.
Health Tips
Never stop the syrup as soon as stools pass, as early stopping causes relapse. Do not force or scold the child while she strains. Encourage physical activity and plenty of fluids. See a pediatrician if there is blood in stools, weight loss, vomiting, or if constipation started from birth.

I understand your concern. What your 2-year-old is facing is very common at this age and is usually due to functional constipation, not a serious disease. What is happening: • A hard stool causes pain → child starts holding stool • Holding makes stool harder → more pain next time • This becomes a cycle, so stools come only every 3–5 days with struggle Why syrups may not seem to work: • Medicines alone cannot help unless diet, water, and toilet routine are corrected • If the child is still holding due to fear, stools remain hard What you can do (very important): • Give plenty of water through the day • Reduce excess milk (more than 500 ml/day worsens constipation) • Daily include: • Papaya, pear, apple (not banana daily) • Vegetables, dal, soft rice, home food • Avoid biscuits, bakery items, junk food • Make her sit on potty after meals daily, calmly, without forcing • Never scold—encourage and praise even small attempts This condition improves slowly but does improve with consistency. When to worry: • Poor weight gain • Vomiting, fever • Continuous bleeding or swelling at anus For proper examination and a long-term plan suited to your child, please contact me directly on Practo. This condition is treatable but needs patience . Medicines must be continued long enough . Do not stop stool softeners once stool becomes normal, taper slowly under medical guidance.
