Hormone imbalance
Irregular menses since puberty ,with high levels of 17oh progesterone (NOT high enough as the reference value for classical / non classical adrenal hyperplasia) , high androstenedione , deoxycortisol ,hirsutism .

1yr is pretty long as to have no improvement at all. Hormones related treatment does take time but we should be able to access something positive by now.
Time taken can defer from person to person but not so much if the case is standstill for a year then we need to rethink of the case.
