Hearing Loss
Since an year I am feeling Deaf. As per audiometry and tympanometry report it is showing moderate conductive hearing loss. Right EAR- 45dBHL Left EAR- 45dBHL

If you don’t have any ear discharge then it can be Otosclerosis. Though you haven’t mentioned the Tympanometry report here.
