This treatment is being suggested on bases of the information provided. However I would like to examine & investigate you  in detail. -Control  Hypertension, Diabetes or  metabolic disorder, if there is any. -Any way it may be tried, --. Dolokind Plus (Mankind) [Aceclofenac100 mg +Paracetamol 350 mg] 1 tab. OD & SOS. X 5 days. --. Caldikind plus  (Mankind) 1 tab OD x 10 days. -(You may need help of your local doctor to get these medicines.) --. Fomentation with warm water. Let the part not be exposed to cold. --. Sleep on a hard bed with soft bedding. --. Use no pillow under the head. --. Avoid painful acts & activities. -- .Do mild exercises for the affected part-ie. Neck and shoulders. --.(Take help of physiotherapist or you can visit my YouTube channel Dr. N. C. Gupta and watch 'Me & My Yoga' and 'Yoga Simplified'. --Do not ignore, let it not become beginning of a major problem. --Do ask for a detailed treatment plan. As the patient is having shoulder pain while doing physical activity so one can assume that patient is not having any stiffness which rules out early stages of frozen shoulder. Better to get an MRI shoulder done in this age it is very common to have muscle tear, muscle atrophy other shoulder pathology.

At his age of 59 years, the most common cause of right shoulder pain is Frozen Shoulder. Xrays of patients with Frozen Shoulder are commonly normal. If he indeed has Frozen Shoulder, he should try the following: *Apply hot water bag (10 minutes twice a day) *After applying hot water bag, apply Nobel Gel (if he is not allergic to it) *Tab Flexon (1 tablet after lunch and 1 tablet after dinner for 7 days) (if he is not allergic to it) *Tab Pantakind (40mg) (1 tablet before breakfast in the morning for 7 days) *Stand in front of a wall, keep both palms in front of him on the wall, then raise the palms along the wall as high as possible and bring them down (this exercise needs to be done upto 30-40 times a day)

Do hot water fomentation. Tablet diclofenac sodium 50mg bd for 5 days. Diclofenac gel application Needs physical examination and clinical tests for proper diagnosis

as per x ray your report is normal. commonly the age related damage can be seen in the x ray. if he is getting pain on doing work or complete movement restriction is there. this can be because of underlying diabetes or muscle tear in shoulder. get proper evaluation done and take treatment correctly.
