Pain in the upper back near the spine with tightness around the shoulder blades/chest during certain stretches is commonly related to muscle strain, posture, prolonged sitting/studying, or tight chest and upper-back muscles. However, it is not possible to confirm the cause without an examination. For now, avoid movements that clearly trigger the pain, use gentle stretching, maintain good sitting posture, and apply a warm pack for 15–20 minutes. Avoid heavy gym exercises or sudden twisting until the pain settles.
Next Steps
If the pain is recurrent, worsening, or persists for more than 1–2 weeks, consult an orthopaedic doctor for examination of the spine, ribs, shoulder blades and chest. Imaging is usually not required initially unless there has been trauma or abnormal findings on examination.
Health Tips
Seek urgent medical evaluation if the chest pain occurs without movement, or is associated with breathlessness, dizziness, fainting, palpitations, fever, severe worsening pain, weakness/numbness, or significant trauma.
