H pylori and sibo
Can upper GI endoscopy detect h pylori and sibo? I was just feeling burps and heaviness at chest. I checked my iron and ferritin a month back because i was feeling hair thinning, it was 38 iron and 22 ferritin and 202 b12, just wanted to know if bad bacteria is causing all this?

Upper-GI endoscopy can detect H. pylori only when gastric biopsies are taken for rapid urease testing or histopathology; visualization alone cannot confirm it. Routine endoscopy does not diagnose SIBO. SIBO is usually assessed with a glucose or lactulose hydrogen–methane breath test; small-bowel aspirate culture during endoscopy is invasive and rarely required Ferritin 22 ng/mL suggests reduced iron stores, while vitamin B12 202 pg/mL is borderline low. “Serum iron 38” cannot be interpreted without units, TIBC and transferrin saturation. H. pylori can contribute to iron/B12 deficiency, and SIBO may reduce B12 absorption, but these results do not prove either condition

Consider: CBC with MCV/MCH, ferritin, TIBC and transferrin saturation Repeat B12 with folate; methylmalonic acid if available TSH for hair thinning H. pylori stool-antigen or urea-breath test before endoscopy, unless alarm symptoms are present SIBO breath testing only if there is persistent bloating, diarrhoea, abdominal discomfort or clear risk factors

Stop PPIs approximately 2 weeks and antibiotics/bismuth 4 weeks before H. pylori breath or stool testing, after confirming with your doctor, to reduce false-negative results Seek urgent evaluation if chest heaviness is exertional or associated with breathlessness, sweating, fainting, arm/jaw pain, or if there is difficulty swallowing, vomiting, black stools, weight loss or significant anaemia

Endoscopy directly can’t detect H. Pylori We can take biopsy and detect that Complete history to be assessed before proceeding to endoscopy

1. Yes. Endoscopy can detect H. pylori 2. Small intestinal bacterial overgrowth  is diagnosed primarily through non-invasive breath testing, which measures hydrogen and methane gases. It's diagnosis is more of clinical. 3. Burps and chest heaviness can be due to gastritis or H pylori infection. 4. Your iron , ferritin and B12 all are low. Iron deficiency is one of the reason for hair fall. 5. Bad bacteria ( malabsorption) and dietary low iron and B12 can cause this.

Gi endoscopy can only detect H.pylori not SIBO which can be diagnosed using a hydrogen breath test. For proper consultation connect on practo.

SIBO causing oesophagitis can be detected by endoscopy but suspected SIBO is not an indication for endoscopy. You should concentrate on treatment of Iron and B 12 deficiency.

Upper GI endoscopy can detect h.pylori but, for SIBO hydrogen/methane test required. Chest heaviness and burps are usually associated with GERD, gastritis. H.pylori can cause low vitamin levels but donot cause hair thinning. For borderline low ferritin and b12 levels, consult a physician to get it corrected
