It can be a combination of causes leading to GERD. Most impirtant is a weak esophagial sphincter which leaks gastric contents in the food pipe above. A hypokinetic bowel which is associated with diseases like diabetes and obesity does not move forward normally and over stay of food in the stomach tends to get back in esophagus during peristaltic movements of stomach. Hiatus hernia if present adds to this problem. Hyperacidity per se is not the cause of GERD although its symptoms can be like reflux esophagitis. Gastroesophagoscopy can pin the exact picture. If esophagial mucosa is not inflammed, it cannot be GERD.
