There are mainly 4 types of Thyroid carcinoma... Papillary Follicular Medullary and Anaplastic... FNAC can clearly identify Papillary Carcinoma Medullary Carcinoma and Anaplastic Carcinoma of thyroid in most cases..... Only Follicular adenoma and follicular carcinoma cannot be differentiated on FNAC...
Next Steps
Depends on your particular problem.. If you have a follicular adenoma also you need surgery.... only the type of surgery will be different.. In adenoma half of the thyroid (one lobe) will be removed whereas in carcinoma the full gland will be removed.

Based on capsular or vascular invasion only carcinoma will be differentiated from adenoma. If you taken out biopsy or full thyroid specimen then only pathologist can able to see the capsule or vascular invasion. In  FNAC, we can't find capsule or vessel. That's why FNAC can't differentiate adenoma vs carcinoma ( this is only thyroid cancer, not related to any other cancer)
