Paediatric fevers start with high temperatures. You are already doing right thing by giving ibugesic and tepid sponging.
We can add mefenamic acid syrup. But in paediatric population dosing is always based on weight of the child. It would have been nice if you have provided weight as well.

You are already doing the right things â ibuprofen (ibugesic) every 6 hours and lukewarm sponging when temperature is high. To help control fever better, you can alternate ibuprofen with paracetamol (Calpol/Crocin) â give paracetamol 3 hours after ibuprofen, and ibuprofen 3 hours after paracetamol, so she is getting an antipyretic every 3 hours. Always give ibuprofen with food to protect the stomach. Ensure she drinks plenty of fluids â water, ORS, coconut water, dal water, or her favourite juice. Watch carefully for danger signs: temperature going above 104°F, febrile seizures (jerking), extreme drowsiness, difficulty breathing, a rash, or if she stops passing urine â go to the emergency immediately if any of these occur.
Next Steps
If fever does not come down by tomorrow (day 3) or any new symptoms appear (rash, abdominal pain, vomiting), take her to a paediatrician immediately â in Bangalore, dengue fever should be ruled out with a blood test if fever persists beyond 2â3 days. Please consult me directly on Practo for a detailed evaluation.
