Dear sir how long you are suffering from extreme back pain?  Any history of injury or history of lifting heavy weights Most commonly the sudden extreme back pain is secondary to muscle strain, which will get better by medicines and bedrest If there is a history of injury, then better to get x ray If there is associated fever, that also needs evaluation If no injury and no fever and short duration pain will get better with medicines and bed rest If pain is persistent then better to consult a spine surgeon
