Your sugar levels are significantly elevated — fasting 264 mg/dL and post-meal 374 mg/dL are well above the diabetic range. Please don't panic, but this needs medical evaluation promptly rather than waiting or trying home remedies. You should get an HbA1c and repeat laboratory glucose test to assess how long your sugars have been elevated and to confirm the diagnosis if required. Since your levels are quite high, a doctor should also assess your kidney function, electrolytes, urine, blood pressure and other diabetes-related risk factors and decide whether you need tablets, insulin, or another treatment based on your clinical condition. If you have excessive thirst/urination, vomiting, abdominal pain, marked weakness, drowsiness/confusion, rapid/deep breathing or dehydration, please go to an emergency department immediately, as very high glucose can occasionally be associated with serious metabolic complications.
Next Steps
See a physician/diabetologist within 24–48 hours. Get: HbA1c, fasting/venous plasma glucose, CBC, RFT/eGFR, electrolytes, urine routine, urine ketones (especially if symptomatic), lipid profile and liver function tests. Further testing can be decided after examination.
Health Tips
Until reviewed, avoid sugary drinks, sweets and large amounts of refined carbohydrates. Stay adequately hydrated unless you have been advised to restrict fluids. Do not start insulin or diabetes tablets on your own, because the appropriate treatment depends on your symptoms, HbA1c, renal function and whether there is evidence of ketosis.

Your sugar readings are quite high — Fasting 264 mg/dL and PP 374 mg/dL. Please don’t panic, but don’t ignore these readings either. For now: 1. Arrange a medical consultation at the earliest so that we can assess the cause and decide whether medication needs to be started/adjusted. 2. Please get HbA1c, fasting/PP glucose and renal function (serum creatinine/eGFR). 3. Please also get a urine examination, preferably including Urine Routine & Microscopy + Urine Albumin/Creatinine Ratio (UACR). This helps us look for protein/albumin leakage and assess early kidney involvement related to diabetes. 4. Do not stop or change any diabetes medication on your own. 5. Until reviewed, avoid sugary drinks, sweets, desserts, juices and refined carbohydrates such as large portions of rice, maida products and bakery foods. 6. Stay adequately hydrated with water, unless you have been specifically advised to restrict fluids. 7. Keep a simple glucose log — fasting and 2 hours after your main meal — rather than checking randomly many times a day. Please arrange the consultation soon. These readings are significantly elevated and need proper assessment rather than trying to manage them with diet alone.
