You have to mention the duration, predisposing- precipitating- perpetuating factors, associated physical illnesses, presence of premenstrual symptoms, seasonal association etc to make an apparent diagnosis and to advise about the management. Anyway most of the symptoms you mentioned are usually that of depression and definitely needs a psychiatric consultation if it's persistent and pervasive.
