You need a professional cleaning done by a dentist. And something for the sensitive teeth can be prescribed as well.

You need to do a general cleaning for your teeth... And you need to use chlorhexidine mouthwash twice daily after cleaning, so that you prevent this calculus formation.. And also a prescribed anti-sensitivity paste...
Health Tips
Do brush twice daily even on the inner surfaces of your teeth and also do flossing...

Get your scaling and polishing done. Use sensodyne rapid relief or any other sensitivity toothpaste to reduce sensitivity which is due to your gums receding due to calculus
