Hi — thank you for explaining your symptoms so clearly. From what you’ve described — high fever with joint pains in wrists and feet that temporarily improved and then returned — this sounds very much like a viral fever with possible chikungunya or dengue infection, which are quite common right now. Let me explain step by step Most likely causes Based on your pattern: Sudden high fever Severe joint pains (especially wrists, ankles, feet) Fever improving then coming back again The two most likely possibilities are: 1. Chikungunya fever – Classically causes severe joint and foot pain, stiffness, and fatigue even after fever settles. 2. Dengue fever (early stage) – May present initially with body/joint pain and fluctuating fever. Sometimes even post-viral reactive arthritis (joint inflammation after infection) can cause lingering pain once fever subsides. Tests you should do immediately Please visit a lab and get these tests done: 1. CBC (Complete Blood Count) 2. Platelet count 3. Dengue NS1 antigen & IgM 4. Chikungunya IgM antibody test 5. LFT (Liver function test) These will help identify the cause and guide the next step. What you can do right now 1. Tab. Paracetamol 650 mg every 6 hours if fever >100°F. Do NOT take ibuprofen, diclofenac, or other painkillers until dengue is ruled out. 2. Hydration is key: Drink 3–4 liters of fluids/day — coconut water, soups, ORS, lemon water. Avoid tea, coffee, and heavy meals. 3. Rest completely. Do not do any physical activity till fever stops for 2 days. 4. Warm compress on painful joints; avoid cold packs. Go to hospital immediately if you notice: Fever >103°F not reducing Vomiting or abdominal pain Bleeding gums, red spots on body, or black stool Drowsiness or irritability These are warning signs — you’ll need urgent evaluation if they appear. Reassurance Don’t panic — this is most likely a viral fever, which usually resolves in 5–7 days, though joint pain can last longer in chikungunya. You’ve done the right thing by taking paracetamol and seeing a doctor early. The next step is to confirm the diagnosis through tests — once we know the cause, I’ll guide you with the exact medicines and recovery plan to prevent recurrence. I can review your reports and symptoms day-by-day to make sure it’s not turning into dengue or a post-viral complication — and tell you when to safely stop medicines. You can message me directly on WhatsApp at nine three two six zero two zero five three six — send me your current temperature trend, lab reports (if done), and list of medicines your doctor prescribed. I’ll help you step-by-step to control your fever quickly, protect your platelets, and ease your joint pain safely.

If fever is still present, after 3 days, proceed with blood tests for dengue, malaria and chikungunya. If the fever not now subsided, but there are symptoms of severe fatigue/ muscle pain, rashes along the body, severe joint pain, pain along or behind the eyes, nausea vomiting, stomach pain, reduced urine output, sudden onset of bleeding from gums, redness in eyes, significant difficulty in doing daily activities, consult a general physician/ practioner at a clinic or hospital, for blood tests. Continue with T. PARACETAMOL 650 mg three to four times a day at an interval of 8 hours till fever relieved.
Next Steps
Suggested blood test for fever of more than 3 days, or severe symptoms:- - CBC - Thick and thin peripheral smear to look for malaria parasite - If less than 5 days, for dengue go for Dengue NS1 Antigen, if more than 5 days, Dengue Ig M and Ig G, NS1 Antigen testing - Malaria antigen test, and chikungunya Ig G and Ig M test
Health Tips
- Maintain adequate hydration, 1.5 to 2 Lt. per day - Keep monitoring temperature if repeated fever episodes, a fever of more than 100° F or complaints of severe symptoms affecting daily activities, visit a nearby clinic or hospital - Take light small meals
