I have had comedonal acne (tiny white bumps) on my cheeks and chin since 2023. Initially, a dermatologist advised me to apply tretinoin pinpoint using a toothpick on each comedone. This made my condition worse and caused multiple breakouts. About a year later, I consulted another doctor who prescribed a salicylic acid–based foaming cleanser and a niacinamide serum (Blemguard). These products helped reduce active acne, but the small white bumps/comedones have still not cleared. I am looking for guidance on how to effectively treat the persistent bumps and prevent further breakouts and help with textured skin. Attaching the pictures below

Acne or pimples... Due to hormonal changes..Oily skin causes it...Common in adolescent age...May occur in adults also.. Food like Oily foods, ice cream, chocolate and sweets increase it.. Comedonal acne have many options for treatment. Tretin won't increase the breakouts. Comedones form due to clogging of pores with oil, bacteria and dust. Follow a good skin care routine.kindly consult for better diagnosis and management.
Hello you ! Such grade of acne is usually treated with topical retinoids (which could be advised only under consultation prescription). Your face skin needs a proper skin barrier repair along with the treatment of the acne.
Spot treatment for tretinoin is not an effective method at all. I understand why this plan has not worked for you or it doesn’t work for anyone in general. There’s a proper way of topical tretinoin application; saying that tretinoin doesn’t work for all skin types and hence there are much soother topical retinoids which can only be advised on doctor consultation.
