I have Chronic thoracic back pain right side deep under right shoulder blade. Pain is constant & consistent from last 15 yrs. Pain starts immediately with sitting position - sitting on car seat, sitting on soft/easy couch, while sitting on western toilet seat wth hand on my knees with slouching/leaning in forward direction.. this is dull achy but constant pain from last 15 yrs.. I am doing intense exercises as well, it gets better for a while but then comes back quickly..there is 1 particular point in between my right shoulder blade & spine wher it pains when pressed.. could you please suggest permanent solution of this & what is the cause of it & what is the permanent solution of this. Sometimes pain is too much. It feels much better when I wake up in the morning but as soon as I wake up & start my day, i feel that feeling this thoracic back pain the same spot & then radiates to my whole right side thoracic (upper right) back & feeling more pain at the centre point where pain occurs

Quite likely pain is from thoracic nerves that exit the spine. No red flags in your symptoms which is reassuring.
*For pains the options would be Lidocaine 5% plaster applied directly on the painful area (Brand name is LIDOVALOR) and should be kept for 12hrs and then removed. So basically 12hrs On and 12hrs off
*Tablet option would be Pregabalin 75mg twice a day and try for a month before knowing if it helps or not.
*TENS MACHINE - quite good and can be found on Amazon etc
*Acupuncture treatment is also worth exploring.
Consider getting an MRI spine to see what is pressing on the thoracic spinal nerves.
All the best.

History given by you suggest a chronic myofascial pain due to improper posture and heavy exercises along with neuropathic pain as it radiating sometimes.
Next Steps
You should go for physiotherapy and along with start taking medicines Tab DOLOKIND MR -BD and Cap PREGABA-M 75 -1 HS. Took these medicines atleast for 10 days along with physiotherapy.
Health Tips
PREGABA -M slightly may cause dizziness in few persons when they wake up....So don't wake up suddenly from the bed in midnight and in morning.
This pattern suggests more of a chronic mechanical/postural thoracic pain with a myofascial trigger point, especially because:
pain is present for many years
it starts/worsens immediately on sitting and slouching
there is one specific tender point between the spine and right shoulder blade
exercise gives temporary relief
morning is relatively better, then pain increases during the day
Possible causes include:
chronic myofascial pain / trigger point in rhomboid or parascapular muscles
postural strain due to prolonged sitting, car seat posture, soft couch posture
scapular muscle imbalance
sometimes cervical/thoracic spine–related referred pain also needs to be ruled out if symptoms persist
Permanent solution usually is not one medicine alone. It requires correction of the cause:
posture correction during sitting
avoiding slouching and forward bending posture
proper back support in car seat and chair
targeted physiotherapy for rhomboids, trapezius, scapular stabilizers
stretching + strengthening exercises
trigger point release/manual therapy if needed
One medicine for temporary symptomatic relief:
Tab Paracetamol 650 mg after food, only when pain is more, up to 2–3 times a day if needed
avoid excess dosing
if you have liver disease, consult before taking it
What I would advise next:
clinical examination by a physician/orthopedic/spine specialist
if pain is very localized and reproducible on pressure, physiotherapy assessment is very useful
if there is radiation, numbness, weakness, night pain, fever, weight loss, cough, or breathing-related pain, then further evaluation is needed
In most such long-standing cases, the best long-term relief comes from ergonomics + supervised physiotherapy + trigger point treatment, not repeated painkillers.

Your description is most consistent with chronic, postureâdependent mechanical/myofascial spinal pain, but we must still exclude more serious causes.
The 15âyear course without systemic symptoms makes malignancy, infection, or inflammatory spondyloarthropathy less likely, but âred flagsâ still need to be checked
Next Steps
take proper consultation for Targeted clinical assessment and management

Please consult a General Physician and Orthopedic surgeon in your area for a physical examination. If no abnormalities are found on clinical examination then you may need an MRI Chest.
No opinion can be given based on your symptoms alone.
You may need a dorsal spine x-ray to check the alignment of the bones. It could be a radicular pain . Treatment depends on the severity and type of pain by consulting and orthopedist or pain medicine specialists
upper back pain like this is usually due to posture.
Correct your posture with a brace along with targeted physiotherapy for long term relief.
If it persists, you may need Orthopedic Consultation for physical examination
