Chest pain that comes and goes any time (day or night), feels heavy in the chest, and gives a strong fear of dying or heart attack is very scary — but the cause is often not a heart attack, especially if you are young, no major risk factors (smoking, diabetes, high BP, family history of early heart disease), and pain is not always with exercise.
Most common reasons for this exact feeling:
• Anxiety / panic attacks (very frequent cause — chest tightness, fear of dying, heavy feeling, comes suddenly, lasts minutes to hours, goes away)
• Acid reflux / GERD (stomach acid comes up → burning/heavy chest pain, worse lying down or at night)
• Muscle strain or costochondritis (chest wall inflammation/pain from posture, stress, coughing)
• Gas / bloating trapped in chest
• Less commonly: heart-related (angina, arrhythmia), lung issue, or pericarditis
The “I am going to die” feeling is classic for panic/anxiety — many people describe it exactly like this even when the heart is fine
Next Steps
• See a doctor soon (within 2–3 days, or today if possible) — better to get checked and remove worry.
• Start with general physician or cardiologist.
• They will likely do:
• ECG (heart tracing)
• Blood tests (troponin for heart damage, sugar, cholesterol, thyroid)
• Possibly chest X-ray or echo if needed
• Go to emergency / hospital right now if:
• Pain is very severe
• Comes with sweating, nausea, vomiting, shortness of breath, pain spreading to jaw/arm/back
• You feel faint or heart racing very fast
Health Tips
• When pain starts: Sit up, take slow deep breaths (in 4 seconds, hold 4, out 6) — this helps a lot if it’s anxiety.
• Try an antacid (like Gaviscon, Eno, or Maalox) after meals or when pain comes — if it improves quickly, likely acid/reflux.
• Avoid lying flat right after eating, reduce spicy/oily food, caffeine, late night meals.
• Do not ignore it — even if anxiety, getting normal tests gives huge relief and stops the fear cycle.
• Keep note: when pain comes, how long, what you ate/did before, how it goes — very useful for doctor.
This feeling is common and treatable — most people find it’s anxiety, reflux, or muscle, not heart attack.
But please get examined soon for safety and peace of mind.
