It is better to have surgery sooner. You definitely need surgery for symptomatic complex tear.

At 32, you should definately go for surgery as menisci will trouble. Although you can still try quadriceps exercises(static) and ice pack application with a knee brace on for 3-4 weeks to allow for ACL and meniscus to heal. Following which you can opt for surgery if you have pain or locking.

Greetings ! as per your history and radiological examinations, you have a complex meniscal tear and grade II ACL injury probably. The meniscal tear requires emergent intervention if the knee is locked in certain position/you cannot fully straighten the knee. ACL shall be arthroscopically evaluated at the time of meniscal injury. for now consult your orthopedic doctor and meanwhile do ice packs and take anti-inflammatory and anlagesic medications for pain and swelling relief. rest to affected part is must. Tab Chymoral plus thrice a day tab Pan 40 once before breakfast would suffice.
