It looks like fungal infection Take tab cetrizine 5mg one morning and one evening for 10 days. Apply candid b ointment twice daily for 14 days and share the outcome. If improvement, candid b  should be continued for 6 to 8 weeks.

Based on your symptoms and the image provided, this appears more likely to be intertrigo/fungal infection (tinea cruris extending to the perianal region) or irritant dermatitis due to moisture, sweating, and friction For a more accurate treatment plan, share: Duration of symptoms Whether itching is worse at night Presence of rash, scaling, or redness Any history of diabetes, recent antibiotics, or similar episodes in the past.
Next Steps
Keep the area clean and completely dry. Wear loose-fitting cotton underwear and avoid prolonged sweating. Avoid scratching, harsh soaps, antiseptics, or steroid-containing creams without medical advice. Use an antifungal cream such as clotrimazole 1% or luliconazole 1%
Health Tips
If you notice night-time itching, bleeding, pain, discharge, or persistent symptoms despite treatment, further evaluation is recommended to rule out conditions such as pinworm infestation, eczema, hemorrhoids, or other anorectal disorders.
