Hi, I am Dr Atul Kumar Nimawat MS ENT.
A 5-year-old child presenting with anterior nasal bleeding is most likely experiencing epistaxis due to common causes such as digital trauma (nose picking), mucosal dryness, upper respiratory tract infections, allergic rhinitis, or local irritation. Less commonly, underlying coagulopathies or vascular malformations may also contribute.
Next Steps
1. Initial Assessment
• Check airway, breathing, circulation.
• Brief history and gentle nasal examination.
2. First-Line Management
• Sit child upright, lean forward.
• Pinch nostrils for 5–10 minutes.
• Apply cold compress over nose/neck.
• Nasal decongestant drops (e.g., oxymetazoline).
3. Local Measures
• Identify bleeding point (Little’s area).
• Cauterize with silver nitrate if needed.
4. Nasal Packing
• Anterior nasal packing if bleeding persists.
5. Treat Underlying Cause
• Manage dryness, infections, or allergies.
• Avoid nasal trauma.
6. Investigations (if indicated)
• CBC, PT/INR, bleeding profile.
7. Follow-Up and Prevention
• Educate on nasal care and hygiene.
• Use saline drops or lubricating ointment if dry nose.
