Yellow teeth has got plaque a thin film formed on the teeth and there might be a calcified , stonelike structure called the tartar or calculus ,which are not good for oral health so it requires cleaning by the therapist.
The therapist or dentist will remove it with or without medication depending upon the condition of the gums & oral hygiene.

Bad breath or halitosis can be due to a wide variety of causes both local and systemic. However the most common cause is poor oral hygiene. This leads to plaque ,calculus,gum disease all of which lead to halitosis.
So as a first step I suggest you visit a dentist to get tooth cleaning(scaling) done followed by polishing of your teeth. The dentist will also advise you on the oral hygiene procedures to be followed including proper brushing technique.
