Hello,
I am Dr. Romain Rajan, General Physician (Ex Govt Medical Officer at RML Hospital, New Delhi), and your case has been allotted to me.
Thank you for sharing your symptoms. I understand how painful and limiting this can feel.
Based on your history of lifting a heavy 20L water can followed by sudden back pain and inability to stand straight, this is most likely an acute muscle spasm or lumbar strain. This is very common and not dangerous, but it can be quite painful for a few days.
What is happening
⢠Sudden load caused muscle strain or spasm in lower back
⢠Muscles tighten to protect the spine, which is why you are bending and unable to straighten
⢠This is a protective response, not permanent damage
What you should do now
⢠Take complete rest for 1 to 2 days (avoid bending or lifting)
⢠Apply hot fomentation 2 to 3 times daily
⢠Use a hard surface for sleeping
⢠Avoid sudden movements or twisting
Medicines
Short term medicines usually help a lot:
⢠Pain relief tablet
⢠Muscle relaxant
⢠Sometimes a short course of nerve relaxant if needed
(These should ideally be taken after consultation)
What NOT to do
⢠Do not try to forcefully straighten your back
⢠Do not do exercises or stretching in acute pain phase
⢠Avoid massage in the first 2 to 3 days
When to start recovery
After pain reduces:
⢠Start gentle stretching and back strengthening
⢠Gradually return to normal activity
When to consult urgently
⢠Pain radiates to leg
⢠Numbness or tingling in legs
⢠Difficulty in urination or bowel control
⢠Pain not improving in 5 to 7 days
Important reassurance
Most cases like this improve within 3 to 7 days with proper care. You will be able to stand straight again once the spasm relaxes.

Lower back pain is due to the acute muscles/ligament strain after lifting the water bottle, resulting in local muscles spasm.Take zerodol p twice daily and a muscle relaxant twice daily.Apply cold water compress. It may take a day or two or more depending on the severity of the spasm
Sudden lower back pain after lifting heavy weight is most likely due to acute muscle strain or ligament spasm in the lumbar spine.
The muscles around the spine go into protective spasm, which makes the back bend forward and difficult to straighten.
Next Steps
• Tab Aceclofenac + Thiocolchicoside – twice daily after food for 3–5 days
• Tab Pantoprazole 40 mg – once daily before breakfast for 5 days
Hot fomentation – 15 minutes, 3–4 times daily
If pain persists >1 week take
• X-ray lumbosacral spine
Health Tips
• Take 2–3 days relative rest but avoid complete bed rest
• Avoid lifting heavy objects or bending forward
• Sleep on firm mattress
• Start gentle back stretching exercises after pain reduces.

This is usually muscle spasms as you have a history of heavy weight lifting.. so you better use a muscle relaxant that could give relief .followed by back muscles strengthening exercises
