Getting shoulder and back pain regularly even for the small holdings and sitting have been suffering from so many days.

It can be due to cervical postural pain leading to nerve compression and further leads to pain in the shoulder, cervical region and down the back. It generally occurs in prolonged posture assumption like long computer work/studies or travelling.. Sometimes there can be any tingling /numbness /heaviness /problem in gripping.
Next Steps
Go to a physiotherapist.. Ultrasonic therapy +IFT +Heating therapy +some strengthening as well as  exercises will be helpful for sure.
Health Tips
At home.. Rest +hot water fomentation +pain killer gel +no long sitting /forward neck bending /travelling /driving +no cold exposure +no manual exertions +no weight lifting +no jerks +correct posture (sit tall).
