Any periodontist here?
I have severe pain in back&distal side of the second molar tooth. No changes after trying SRP. It had tartar buildup, did SRP in May.  Tooth pic taken before SRP. Went back in June and dentist said that tooth has some buildup again and scraped it.Only this tooth developed buildups quicker. What could be the reason? I use floss, electric toothbrush. Dentist gave peridex rinse 2 weeks ago. No changes in pain. It's throbbing surrounding that gum margin and mostly on distal and back side of the Tooth. Went to endodontist and he did CBCT scan. He said no infection but he could see inflammation on the scan. So he suggested root canal. Is this xray shows anything? The distal side of the x-ray looks very dark when compared to other side. Is this infection or any gum issues? Why it's dark on the distal side? Do I need to try RCT?Or extract 47? The same thing happened to#46, it was painful for 3 years.Did RCT and no relief. Retreat RCT several times. Ended up pulling it last July. Do I ext 47?

From your description, it seems that you have been experiencing severe pain in the back and distal side of your second molar tooth (#47). It's essential to follow the advice and recommendations of your dental professionals, including your dentist and endodontist, as they have examined you and have access to your dental history and X-rays. Regarding the X-ray, the appearance of darkness on the distal side of the tooth may indicate different things, such as: Infection: A dark area around the root can indicate the presence of an infection or abscess. Cavities or Decay: Dark areas could also indicate cavities or tooth decay. Bone Loss: Periodontal disease or gum issues could lead to bone loss, which may be visible on the X-ray. Artifacts or Image Quality: Sometimes, X-rays may have artifacts or variations in image quality, leading to areas appearing darker. Given that your endodontist suggested a root canal due to inflammation seen on the CBCT scan, it's possible that there is an issue with the tooth that requires treatment. Root canal treatment is often recommended to address infection or inflammation within the tooth's pulp, which can relieve pain and save the tooth. If you have concerns or doubts about the recommended treatment, you can consider seeking a second opinion from another dental professional. They can review your X-rays, examine your tooth, and provide their expert opinion on the best course of action for your situation. Remember that dental conditions can be complex, and it's essential to work closely with your dental team to address any ongoing pain or issues with your teeth. Prompt treatment can help prevent further complications and promote better oral health. Please consult with your dentist or endodontist to discuss your concerns and to determine the most appropriate treatment plan for your specific case.

The gum around the tooth is inflamed. Also, the distal side has a deep pocket and the food is getting impacted and has resulted in  caries.
Next Steps
This tooth needs root canal and deep curettage around the tooth.
Health Tips
Medication and taking a soft diet is recommended to get immediate relief. Visit to the dentist is required.
