You can go for Assessment of vitamin d 3 also. TSH normal, b12 is good. Mostly these r the reason.

Modalert 200mg is quite a high dose. Feeling sleepy in the day can be due to improper sleep at night. If you are confident that you are giving enough time for sleep, then you must evaluate regarding OSA (obstructive sleep apnea). Do you snore in sleep? Are you obese? Do you have short neck? How sleepy are you during daytime?
