I have a fear of having heart attack out of nowhere, I even sometimes feel the physical symptoms like chest , arm, shoulder and back pain, I am always worried. I also sometimes feel something bad is going to happen to me specially while travelling. I even stopped riding bike because I used to have heart palpitations and skipped beats. I went to a cardiologist all the test echo, ecg and tmt all are fine. But still I think somethings wrong and not been able to diagnose myself wheather it's anxiety or not. Please help.

Your symptoms strongly suggest health anxiety/panic disorder rather than a heart problem, especially since cardiac tests are normal. Anxiety can mimic real physical sensations—chest pain, palpitations, and fear of dying—creating a vicious cycle of worry. Avoiding travel or biking reinforces this fear.
Understanding that these sensations are anxiety-driven is the first step. Practice relaxation techniques, breathing exercises, and gradually face avoided situations. Avoid excessive self-checking or Googling symptoms.
Treatment with therapy, particularly Cognitive Behavioral Therapy (CBT), is very effective. In some cases, medications may be needed. I recommend consulting a psychiatrist for proper diagnosis and structured management.

Health Tips
Please follow up regularly, take medications daily, and share your improvement and any emotional difficulties openly so we can support you better.
Since your ECG, Echo, and TMT are normal, a serious heart problem is less likely. Symptoms like chest pain, palpitations, and fear of sudden heart attack can often occur with anxiety or panic. Try slow deep breathing during symptoms, reduce repeated checking of heart sensations, maintain regular exercise, and limit caffeine. If the fear continues, consulting a psychiatrist can help with proper evaluation and treatment. I understand these sensations feel very real and frightening, but they are commonly seen in anxiety and can improve.
