Hello, I am Dr. Romain Rajan, General Physician.
Your case has been allotted to me.
I understand your concern. A
vitamin B12 level of 160 pg/mL is on the lower side and indicates moderate deficiency, which can cause fatigue, tingling, mood changes, and weakness if not corrected. The good part is that it responds very well to supplementation.
â
Advices
Include eggs, milk, curd, paneer, fish, or chicken in the diet if non-vegetarian; if vegetarian, continue with fortified cereals and milk.
Avoid alcohol and smoking as they reduce absorption of vitamin B12.
Combine supplementation with a balanced diet rich in iron and folate for better response.
Management
You can start one of the following options:
Tablet Methylcobalamin 1500 mcg once daily after breakfast for 8â12 weeks, or
Neurobion Forte tablet once daily after breakfast for 3 months (contains B12 + B6 + B1).
If symptoms like numbness, fatigue, or tingling are marked, your doctor may advise B12 injections â Methylcobalamin 1000 mcg intramuscular once weekly for 4 weeks, then once monthly for maintenance.
What to Monitor
Improvement in energy levels and appetite within 2â3 weeks.
Repeat vitamin B12 levels after 3 months.

If numbness or tingling worsens.
If fatigue or mood issues persist despite supplementation.
Every patientâs requirement differs based on diet and absorption capacity. For optimal dosing and follow-up, itâs best to discuss your history and any concurrent medications in detail.
