This risk is particularly serious in newborns and younger infants whose liver cannot process opioids efficiently. The FDA has issued specific warnings against tramadol use in breastfeeding mothers, and it is classified as contraindicated during lactation.

If you are in pain and need a painkiller while breastfeeding, the safest options are: Paracetamol (Calpol/Crocin) 500-1000 mg, which is very safe for both mother and baby.
