Join a support group and time to face your shame. Don't be guilty or feel fear. Also consult a psychiatrist.

Hi... Why are you calling it a sex addiction? Are you having sex uncontrollably with one or multiple partners? Masturbation is not sex, I suppose you mean, you are masturbating very frequently and feeling addicted to it. For your sexual health related problems you can consult a Psychologist or a Sexologist. For depression related issues, please consult a Psychologist. I suppose your mental health issues along with poor lifestyle is causing this sexual health problems.
Next Steps
Consult a Psychologist. CBT along with Lifestyle Guidance and Sexual Health Counselling is required.
