Low hemoglobin

2025-05-04 08:29:51
Hello Doctor, I’m a bit concerned about my wife's condition. After taking Socril regularly for 3 months as prescribed, her hemoglobin has dropped from 9.4 to 8.9. She has been consistent with the medication and following dietary advice. Could there be a reason for this decrease? Should we consider adjusting the dose or trying a different iron supplement? Also, would you recommend any further tests to understand the cause? Thank you.

Hi, Yes we need to understand why despite supplements her hemoglobin has dropped. Kindly consult with all the reports once. It could be an issue with absorption or there could.be blood loss which is too small to be seen by the naked eye

Can understand your concern. This needs to be evaluated.  There are many causes for anemia. It could be due to nutritional causes, chronic diseases, blood loss or may be inherited too. So just by taking iron alone will not get your anemia corrected if its not caused by iron deficiency. Kindly get a CBC, iron profile, vit b12 and peripheral smear, RFT and stool OB. Kindly connect, requires more detailed history. Shall definitely help you overcome your problem.

If she has been taking the medication in the correct dose and manner (on an empty stomach, without following it with tea, coffee, or other supplements), her hemoglobin should have improved by now. A continued drop in Hb despite three months of supplementation raises concern, with ongoing blood loss being one of the most important causes to consider. Please provide additional details, such as any co-morbidities, history of heavy menstrual bleeding, or any episodes of blood noticed in the stool or urine. Keep in mind that gastrointestinal blood loss is often not obvious. I would recommend doing a peripheral smear, serum ferritin, ESR, urine routine examination, and stool for occult blood. These will help guide further evaluation.

Hello, I understand your concern. A drop in hemoglobin despite regular iron supplementation like Sofcril (iron + folic acid) could indicate one of the following: Poor absorption of oral iron (often due to gastric issues, tea/coffee with meals, or low vitamin C intake) Chronic blood loss (e.g., heavy periods, gastrointestinal issues) Underlying causes like vitamin B12/folate deficiency or chronic conditions Here’s what I suggest: 1. Ensure iron is taken on an empty stomach or with vitamin C (like orange juice) to improve absorption. Avoid tea/coffee within 1 hour of the dose. 2. Further tests may help: CBC with peripheral smear Serum ferritin Vitamin B12 and folate levels Stool for occult blood (if chronic GI loss is suspected) If absorption issues or side effects persist, switching to a different form of iron (e.g., ferrous ascorbate or iron sucrose IV) may be considered after medical evaluation. I recommend a direct consultation to assess the full picture and adjust treatment safely. Take care!
