There are two problems in this case as can be seen in the picture- 1) the natural teeth have got abraded (white portion gone and yellow part of tooth visible) 2) food deposits (calculus as professionals say) on the teeth and at the junction of teeth and gums (this is what is causing the bad breath, bleeding from gums , loose gums etc). Now the 1st problem can be solved by covering your natural teeth with caps (crowns) to stop the further abrasions and for the 2nd problem I.e. bad breath- visit your dentist and get professional cleaning done (as the deposits have got hard and you will not be able to clean that with normal brushing) , after getting all the deposits clean brush properly and regularly twice a day to make sure that there is no white soft layer getting deposited on your teeth , use chlorhexidine mouthwash (or as suggested by your dentist) everyday to minimise the deposits on your teeth. Visit your dentist every 6 months for follow up.
