What you’re describing is more than simply being lazy or unmotivated. Since these difficulties have been present from your early teens and involve attention, procrastination, restlessness, impulsivity and difficulty listening, it would be useful to get a proper ADHD assessment rather than self-diagnosing.
The fact that you can focus deeply on music/singing also doesn’t rule out ADHD—interest and stimulation can strongly affect attention.
I’d suggest taking a psychological assessment and therapy to understand what is actually contributing to these difficulties and learn practical strategies for studies, career and daily functioning.
