Tongue swelling with breathing difficulty, seven times in a year, in someone on Telma 80 is very likely drug-induced angioedema from the telmisartan - not an ordinary food allergy. That is exactly why the antihistamine barely helped and it took three hours to settle: this type of swelling is bradykinin-driven and does not respond well to Alegra. This is a genuinely dangerous pattern, because the next episode can close the airway. Please do not treat it as a minor allergy.

Common Underlying Causes in Senior Adults
:
Medication-Induced: Commonly caused by Blood Pressure medications, especially ACE Inhibitors (e.g., Ramipril, Enalapril) or ARBs (e.g., Telmisartan, Losartan), as well as certain painkillers (NSAIDs).

Severe episodes require emergency administration of Intramuscular Epinephrine (Adrenaline), intravenous corticosteroids, and fast-acting antihistamines under clinical supervision.
